Any person 17 years or younger at the start of camp has to be accompanied by a guardian. The guardian needs to live in the same residence hall as the minor and has to be registered for the camp. The guardian may either be the minor’s parent/legal guardian or an adult they assign to be guardian while at camp. To assign a guardian, a Liability Release, Medical Release and Waiver is required before the minor can be registered. Only a Waiver is required if the minor’s parent is also attending camp.

The Liability Release can be a letter that states that Endicott College is released of any liability while the minor is attending Beantown Camp 2012.

The Medical Release can be a form obtained from a healthcare provider (it will be called something like Authorization for Medical Treatment of Minors). Alternatively it can be a letter that states that the assigned guardian (give their name) is authorized to make emergency medical decisions for treatment by a hospital or physician should an injury occur to the minor. It should include any relevant medical information including healthcare provider details (name, telephone number, policy number). It must be signed by the parent/legal guardian and include their contact information (telephone numbers, address).

Go to the Waiver page to print a copy. It must be signed by a parent/legal guardian on behalf of the minor and sent in advance as well. It cannot be completed at check-in by the minor.

If you do not have someone attending the camp who can act as guardian, please have your parent or legal guardian contact us and we may be able to find an adult who would be willing to do so.
